Care-finder answers
Your selected needs, location, coverage type, and urgency are used inside your browser to display possible agency options. The preview does not send those answers to our server, and it does not request a Medicaid number, Social Security number, diagnosis, or insurance document.
Optional contact prefill
After choosing an agency, you may enter your name and email address or mobile number so those contact details can be passed directly to the agency's IntakeQ form. The care-finder answers are not included. You may skip this feature and open the IntakeQ form without prefill.
Agency intake
The actual intake is hosted by IntakeQ for the selected agency. IntakeQ and the agency govern the information entered there under their own notices, agreements, and security responsibilities. The agency remains responsible for reviewing, responding to, and protecting the intake.
Agency sales requests
The agency quote form is for business contact information only. Do not submit patient, consumer, clinical, or protected health information. A final business email or scheduling destination will be connected before external launch.
Before public launch
Provider participation, licensing or certification, program availability, service areas, payer information, consent language, contracts, security requirements, operator contact information, and data responsibilities must be verified in writing before consumers rely on this service.
