Effective September 3, 2026. This notice explains what The Place for Care collects when someone asks for help finding care.
What we collect
We first collect the basic information needed to save your request: your name, mobile number, care needs, location, coverage type, timing, and agency choice. If you continue, the full intake may also include your date of birth, address, Social Security number, insurance or Medicaid member ID, current care, diagnoses, medications, allergies, support needs, emergency contact, consent, and electronic signature.
Why we collect it
We use this information to respond to your request, help you take a next step, avoid duplicate outreach, track whether you connected with care, improve the service, and show aggregate results. We do not sell your personal information.
Contact permission
We contact you only after you agree that The Place for Care may call or text you about your request. A call may use an automated assistant that identifies itself. Message and data rates may apply. You may ask us to stop at any time. This permission does not authorize unrelated marketing.
Data minimization
We collect a Social Security number only when it is required to prepare the agency application. It must be entered through the protected intake form, not spoken on a call or sent through text, voicemail, or regular email. Staff workspaces show whether it was collected without displaying the number.
One intake
You can complete the full intake on The Place for Care. We use your answers to create one organized record. We share that record with the agency you selected only after you give permission. The agency remains responsible for reviewing it, confirming eligibility, responding, and protecting any copy it receives.
How we protect it
Full intake records are encrypted before storage. The public website cannot list or download them. Private staff access requires a separate owner key, and complete records are separated from public pages. Encryption is one safeguard; required business-associate agreements, access policies, training, risk review, and incident procedures must also be in place before broad production use with protected health information.
Your choices
You may ask us to correct or delete a care request, stop contacting you, or explain what we have stored. A dedicated privacy contact address and request process must be added before a broad public campaign begins.
Agency sales requests
The agency quote form is for business contact information only. Do not submit patient, consumer, clinical, or protected health information. A final business email or scheduling destination will be connected before external launch.
Service limits
Provider participation, licensing or certification, program availability, service areas, payer information, and eligibility must be confirmed with each agency. The Place for Care does not guarantee coverage, admission, housing, an appointment, or care.
