Possible options, not medical advice
The care finder may display agencies based on service, location, and unverified preview-profile coverage information. It does not diagnose, perform clinical triage, or confirm eligibility, insurance benefits, admission, program capacity, appointment availability, assigned staff, housing, or outcomes.
Not an emergency service
Call 911 for immediate danger. Call or text 988 for crisis support. Do not wait for a response through this website during an emergency.
Agency responsibility
Each agency remains responsible for approving public claims, verifying coverage and eligibility, making clinical decisions, obtaining required consent, protecting records, and delivering care.
Directory order
Payment does not determine whether an agency appears first. The pilot uses service, location, coverage information, how many selected needs appear in the preview profile, and alphabetical order. Every participating agency must verify its profile before public launch.
Commercial model
Agency pricing covers fixed technology, campaign, and administrative services defined in a written scope. It is not calculated per referral, admission, Medicaid claim, reimbursement, or enrollment.
Final agreement controls
Website estimates are planning estimates. Final deliverables, limits, timing, ownership, privacy responsibilities, fees, and exclusions are confirmed in a signed agreement.
